IRB Applications, Templates and Resources

If you are unsure about what forms you need, please review the following information:

The templates at the bottom of this page are useful in making sure your supporting documents contain the appropriate content and language. Submitting the proper materials makes the approval process faster and more efficient.

    • focus groups
    • researchers are in a position of authority over subjects
    • plans to collect sensitive information from participants
    • distressing subject matter or procedures, beyond what participants might experience in an average day
    • deception
    • recruitment from vulnerable populations (children, pregnant women, prisoners, or persons with diminished decision-making capacity).

  • . For reporting unanticipated problems during an IRB-approved study. Complete this form within five (5) business days of the occurrence. Contact IRB@stedwards.edu with questions. [This of the application can be used for planning purposes only.]
     
  • . For IRB approval when:
    • an approved research study has changes to its research team or protocols, or
    • an IRB Full Board-approved study needs to continue beyond one year. For researchers with an IRB-approved study from another institution and want to facilitate the same study at St. ·¡»å·É²¹°ù»å’s. If you are a non-St. ·¡»å·É²¹°ù»å’s researcher, you must have a St. ·¡»å·É²¹°ù»å’s co-investigator before applying. [This  of the application can be used for planning purposes only.]
     
  • St. ·¡»å·É²¹°ù»å’s University . If you are not affiliated with St. ·¡»å·É²¹°ù»å’s, use this form to request permission to recruit St. ·¡»å·É²¹°ù»å’s University participants for your study. [This  of the application can be used for planning purposes only.]
